FOSTER/ADOPTION APPLICATION
 
 We are a group of volunteers at the city kill shelters who advocate animals
 in danger and facilitate adoptions often through many major recognized rescue
 groups.  We're also starting a new rescue organization, named DaKoDaLoVe.
 After we meet the animals, we socialize them and get to know them
 to best match them into foster or 4ever homes.

 We ask fosters and adopters to complete and sign the foster / adoption agreement
 and (for adopters only) there is a non-refundable donation of anywhere from
 $150 to $200 depending on the pet and on the Rescue (usually around $150)
 and for puppies 9 months and under the adoption donation is $195,
 which goes towards our/the rescue's costs for vet care and supplies, transport, etc.  We also do a home visit as part of the adoption process.

 The adoption includes spay / neuter surgery, all vaccines, and microchip.
 (all dogs come with the above, and will only go through state lines legally
 and with proper paperwork).  The nominal donation enables the rescue
 to stay afloat and keep saving other animals' lives.
 We do not make any money out of these donations, they barely cover
 a small part of our entire costs, which are simply outrageous between
 medical needs, boarding and transport.

 By adopting a pet and donating the adoption fee you're not only saving one life, you're saving many lives because you enable the rescue to continue their
 work by not letting them get impossibly overwhelmed by expenses.
